In some cases, only minimal support is needed for a client to live as independently and safely as possible. Constellation’s caring Companions and Homemakers lend a client:
- Companionship and conversation
- Monitoring diet and eating, providing medication reminders,
picking up prescriptions - Arranging appointments and appointment reminders
- Stimulating mental awareness, such as assistance with reading
and games - Assistance with walking
- Accompaniment to lunch, dinner
- Houseplant and pet care
- Home maintenance and supervision
- Preparation of grocery lists and proving transportation to the
grocery store - Light housekeeping such as: assistance with laundry and ironing,
making beds and changing linens, dusting furniture and cleaning
floor, taking out garbage - Planning, preparation and cleanup of meals
- Assistance with pet care such as grooming, walking, feeding
- Assistance with morning wake-up and evening tuck-in
- Escort to appointments, shopping, errands
- Ongoing teaching and support of Independent Activities of Daily Living (IADLs)
We understand that making the decision to seek outside help for a loved one is difficult. To help you determine the need for assistance, ask yourself:
- Has your loved one had trouble recently with tasks around the house?
- Have they sustained an injury trying to perform routine chores?
- Do they live alone?
- Has your loved one become more forgetful about medications,
appointments or paying bills? - Have they started to cook or clean less than usual?
