Many times, we don’t realize that the support we need is just a phone call away. Constellation Private Duty services make it possible for a client to remain at home and live as safely and independently as possible. Our experienced team of nurses, certified care assistants, geriatric care managers, companions and homemakers deliver individually tailored professional and personal assistance at competitive, privately paid rates.
Available 24 hours a day, 7 days a week, our private duty services are provided in a variety of settings including the client’s home, assisted living facility, nursing home or hospital. In many instances, private insurances or worker’s compensation programs are accepted.
Our team of private duty caregivers undergo a thorough background check, a full orientation and customer service program and attend educational in-services throughout their employment with Constellation.
Our competency lab, opened in March 2011 ensures that each Certified Care Assistant is qualified to take care of the client when in the field. Employees must pass several field competency tests such as the appropriate way to lift a client in and out of bed, or where to set up a chair in the bathtub.

Private Duty services are customizable to the level of care needed and the appropriate schedule of the client. Services can be arranged by the hour, by the shift or on a live-in basis and are fully customizable to the client’s needs.
