Maximizing Independence

Constellation Home Care offers home-based behavioral health treatment to clients using a holistic and comprehensive approach.  Our visiting behavioral health care staff works with the client’s physician or interdisciplinary care team to develop an individualized plan of care.

The goal of our behavioral health professionals is to empower our clients to reach their highest level of independence. By focusing on personal responsibility and healthy behaviors, education is provided in the areas of physical, cognitive, affective and social functioning to help each person achieve the maximum level of independence within the limitations of their disease process.

Constellation’s behavioral health nurses have extensive education and clinical training in both behavioral health and medical inpatient settings, and the community.

Our Behavioral Health staff is comprised of:

  • Nurses with extensive clinical experience in behavioral health and medical inpatient settings as well as home care.
  • Physical and Occupational Therapists
  • Home Health Aides
  • Social Workers


Services provided:

  • Medication management tailored to client’s individual needs, including medication administration up to twice a day and medication pre-pours
  • Customized education geared to the specific medical, functional and psychosocial needs and limitations of the client
  • Management of client’s medical needs and communication with primary care physicians and specialists
  • Occupational Therapists can be used to determine the appropriate living environment and to assist clients in maximizing independence with ADLs and IADLs
  • Referrals to appropriate community resources

As a licensed home health agency, Constellation Home Care can bill Medicare, Medicaid and private insurances for services.

Referrals welcomed 24/7 and assessments are completed within 24 hours.
