Points of Life

Founded in 1985 as Norwalk Rehabilitation Services/Health and Home Care Resources, the company officially changed its name to Constellation Health Services in 2009 to reflect increased range, both in terms of services offered and geographical reach.

From our beginning as a private outpatient physical and occupational therapy clinic almost 25 years ago, we have evolved into a dynamic, diversified company with almost 300 staff members providing services to people of all ages throughout Connecticut’s Fairfield and New Haven counties.

And while the scope of services provided by Constellation Health Services is wide, our team approach ensures that each and every person, and organization we work with benefits from our combined knowledge and clinical experience.
