Constellation Health Services has a full time opportunity for a Provider Relations Representative to work in southern Fairfield County.

The Provider Relations Representative will be responsible for assessing patients, marketing and generating referrals for our busy Home Care and Private Duty agencies and Geriatric Care Services; and build strong relationships with existing and new referral sources.

• Assessments will be provided in hospitals, long-term care facilities and at other health care sources in southern Fairfield County.
• The ideal candidate will be results driven, highly motivated and have proven customer service skills.
• Health care sales experience, health care marketing, case management and discharge planning experience is preferred.
